The Suffield Parks and Recreation Department is pleased to have you participate in our programs.  We understand the importance of providing high quality programs and services to our residents, and would appreciate hearing from you in regards to your experience in a specific program or activity.

At the completion of most programs, an evaluation form is distributed.  If you have already completed one, Thank You!  If not, and you would like to provide input about a program you or your child recently participated in, please click on the button below, print out the form and return it to the Parks and Recreation Department, 230C Mountain Rd.
